You can fill out the documents ahead of time, but need to go the first night on the ship to the Club O2 orientation. The time will be listed in the Funtimes paper. Basically the counselors are there to collect your forms, put the Club O2 sticker on the teen's sail-n-sign card, and hand out the activity sheet for the week. They have activities scheduled most nights until midnight-1:00 am, and several things on sea days also. The teens can pick and choose what activities they want to attend, they do not sign in/out, just show up. Club O2 has its own club room and the kids hang out in there quite a bit, too, playing video games, etc. My son absolutely loved it, made a lot of friends both cruises he was in it. He would take off right after dinner every night, come in after the activities ended.
