All bars and restaurants in the foreign countries you visited, not to mention quite a lot of the other shops as well, allow smoking. The notion of NOT allowing smoking is unheard of to them, in fact, unless there is some risk of explosion.

This is 2012 and 2013 there should not be any smoking on any boat that holds over 2000 people

Extremely doubtful. Smokers comprise a large portion of Carnival's customer-base, and smokers are, by and large, the bigger gamblers and the bigger drinkers - which are high profit-margin functions of all cruises.
Now, while I share your dislike of tobacco smoke - being around it for more than a few minutes gives me a two-week sinus infection - I do not share your strident prohibitionist piety.
